MVP Health Plan is a not-for-profit IPA model HMO service Dutchess, Ulster and Orange Counties. Our extensive network of private practice physicians work with the HMO to deliver comprehensive routing and preventative benefits at affordable premiums. In addition to our doctors, MVP features:

• No claim forms; No annual deductibles; No waiting for reimbursement; No lifetime maximum
• $25 co-payment for Primary Care Physician Office Visits, $40 for Specialist visits. For maternity coverage, physician charge of $200 co-pay for delivery.

CDPHP is a New York based not-for-profit HMO. Capital District Physicians' Health Plan is comprised of a network of independent physicians located throughout the Hudson Valley.
